Program for Assertive Community Treatment (PACT)

The PACT program at Catholic Charities is a wraparound mental health program that serves chronically mentally ill adults, typically with a diagnosis of schizophrenia, schizoaffective disorder, bipolar disorder, or other psychotic disorders. “Wraparound” means professionals from several different disciplines come together to provide you the care and services you need to manage your diagnosis and live a fulfilling life. The PACT team provides on-call crisis services (including face-to-face crisis assessment and intervention, when necessary) 24 hours a day.

The PACT team may include: team leader, nurse, peer counselor, chemical dependency specialist, vocational specialist, medication manager, and others. The PACT team meets you at home, work, or out in the community—anywhere you may need a little extra assistance. That could include bringing you medication twice a day, providing you transportation to a job, or simply helping you navigate the tasks of daily living.

The PACT team also works with a Stakeholder Advisory committee of mental health consumers and family members to ensure a thorough understanding of the challenges facing individuals with chronic mental illness.
